How does this sound - (bearing in mind Melody's suggestion of keeping it polite)
Dear ******,
I am a little worried that I haven't heard from you regarding your Exchange Gift that I posted to you a couple of weeks ago. I see from the tracking on the parcel that it arrived safely on the 25th July.
I wondered whether there was a problem with it? Perhaps the colour is wrong or you don't like it? I gave it a lot of thought before starting your gift and it was the comment on your questionnaire "Anything particular that you would like to receive? - A gorgeous fine shawl " That got me thinking that it would be a lovely surprise for you to receive one.
If you are really not happy with it, it would be no problem for me to knit something else for you, if you wanted to return it...

I'm another that agrees with Pictish - you really don't want to be offering to knit anything else for her!
You could maybe put a line in the last part saying that you always ask for feedback whenever you take part in swaps, so that you get a picture of what people have liked or not liked about things you've made. That way it will sound like it's a more general thing that you do on all swaps rather than something personally aimed at her? _________________

Exactly.
I agree that the email is perfect but I'd take out the knit something else as well
I wasn't trying to say that you wouldn't be polite by the way.... it's just that sometimes being overly polite gets the message across much better than sounding cross iyswim?? I wasn't meaning that you would be cross either!! Oh dear....
